In January 2026, obtaining an official Health Certificate (CVI) requires paying a fee. We have been blessed by the generosity of the Webster Vet Clinic who have inspected our animals and provided an official CVI for many years at no charge to 4-H families. The Webster Vet Clinic wants to continue providing a free service to 4-H livestock youth and ensure the health of all animals at the DCF. We worked together and developed the DC Health Information Form to be used for all animals that will be at the DCF.

What is the DC Health Information Form?
This form is a COUNTY ONLY equivalent for an official CVI. This form will only be accepted at the Day County Fair (DCF). Animals must pass a health inspection by a veterinarian to be exhibited and stalled at the DCF.

Is the DC Health Information Form the same as a Health Certificate (CVI)?
No, the Health Information Form is only for Day County and will only be accepted at the Webster Vet Clinic.
Do I have to use the DC Health Information Form?
No, you can work with your veterinarian to obtain an official Health Certificate (CVI) and pay the fee required.
What are the steps I need to take to use the DC Health Information Form?
1. Fill out the form(s) with all your animals’ information BEFORE going to the Webster Vet Clinic August 5-6. Your animals will not be inspected until the form is completed.
2. Bring your animals and completed form to the vet clinic.
3. Hand your form to the vet staff to fill out as they inspect your animals.
4. Thank the Webster Vet Staff for their support of Day County 4-H.
Can I get an official CVI for the State Fair at the same time?
Animals going to the State Fair are required to have an official CVI. They are good for 30 days and are destination specific. Refer to the State Fair book for livestock check in times to be sure that your official CVI will fall within that 30 day time frame.

4‑H is a community that provides experiences where young people learn by doing. Kids complete hands-on projects in areas like health, science, agriculture and citizenship, in a positive environment where they receive guidance from adult mentors and are encouraged to take on proactive leadership roles. Kids experience 4‑H through in-school and after-school programs, school and community clubs and 4‑H camps. We recognize that the youth of today are our future leaders and what they learn and experience in 4-H will help them develop into contributing citizens who will give back to their communities in the future.
