Day County Certificate of Compliance support

 Please use button, link or QR Code below to input information into a google form for the Certificate of Compliance (CoC). This is required for all 4-H events, meetings, clinics or practices you as leaders/volunteers are planning. Information needs to be input a minimum of 5 days prior to event to ensure proper filing of CoC.
